Nurse Supervisor Home Care
Responsibilities:
- Completes initial comprehensive and ongoing assessment of patient and family to determine best services to meet their needs
- Coordination of services with the patient, family, and care giver team.
- Ongoing supervision of all professional and para-professional staff assigned to the patient.
- Ongoing communication with patient and family to ensure client satisfaction and best outcomes
Requirements:
- Registered Nurse (RN) license NYS
- BSN required
- 3 + years of clinical experience
- Home care experience
- Minimum 1 year of supervisory experience
- Excellent observation, problem solving and communication skills
- Compassion and integrity
